STUDIO EXPERIENCE PARTICIPATION & FILMING WAIVER

Please complete this form before taking part in a Studio Football experience. For participants under 18, the form must be completed by a parent or legal guardian. Please complete one form for each participant.

2. PARTICIPATION, SAFETY & ACCEPTANCE OF RISK

Football, drills, challenges, physical testing and associated activities are physical sporting activities with inherent risks that cannot be completely eliminated. These can include slips, trips, falls, loss of footing, collisions, physical contact, being struck by a football, strains, sprains, contact with goals or equipment and other ordinary risks associated with football.

Studio Football takes reasonable measures to identify and reduce foreseeable risks, including regular visual pitch and equipment inspections and active management of participants during filming. Reasonable precautions cannot remove every inherent risk of football.

I understand that taking part in football and related physical activities involves inherent and ordinary risks associated with football and physical sporting activity. I voluntarily participate, or permit the participant named above to participate, on that basis.I agree that the participant will follow reasonable safety, coaching and filming instructions; wear suitable footwear and clothing; use the pitch and equipment appropriately; avoid reckless behaviour; report hazards or damaged equipment; and stop and inform Studio Football if injured or if continuing becomes unsafe. Participants must not interfere with cameras, tripods, lighting, cables or filming equipment unless instructed. I understand that Studio Football may modify or stop an activity or a participant’s involvement where reasonably necessary for safety.I confirm, to the best of my knowledge, that the participant is fit to take part and that I have disclosed any relevant medical condition, injury, allergy or other information that Studio Football should reasonably know about for safe participation.I understand that alcohol and illegal drug use is strictly prohibited prior to or during participation in a Studio Football session. Studio Football may refuse or stop participation where it reasonably suspects that a participant is under the influence of alcohol or illegal drugs.

3. MEDICAL INFORMATION

Medical conditions, allergies or other relevant informationI explicitly consent to Studio Football Limited collecting and using the health, injury, allergy or other relevant medical information I provide above solely where necessary for participant safety, reasonable adjustments and emergency response, in accordance with Studio Football’s Privacy Notice.

4. UNDER-18 SUPERVISION

This route is shown only where the participant is under 18.

Where a permitted group-session supervision arrangement is used, the form records the authorised responsible adult’s details and confirmation. For individual experiences, the parent/legal guardian is required to remain on site.

Where applicable, names / authorisation for any other children the completing adult is responsible forI confirm that the applicable supervision details provided are accurate and that Studio Football will be informed promptly if the arrangement changes.

5. FILMING AS PART OF THE EXPERIENCE

Filming and photography are integral to Studio Football experiences and cameras and filming equipment will be present. Where filming is necessary to provide the booked experience, Studio Football may record the participant and edit and deliver the agreed content to the booking customer. During group activities, other participants may appear incidentally in customer-delivered footage.

I understand and accept the participation and core filming arrangements above. I understand that I/the participant cannot take part in a filmed Studio Football experience without accepting the filming necessary to provide that experience.

6. AI TRACKING AND FILMING LIMITATIONS

Studio Football uses automated AI tracking equipment as part of some filming setups. While this technology is designed to follow play and capture action, Studio Football cannot guarantee 100% detection or capture of every ball touch, goal or individual moment.

7. OPTIONAL STUDIO FOOTBALL MARKETING PERMISSION

This choice is separate from the filming required to provide the booked experience. Saying NO does not prevent participation.

YES: I give Studio Football permission to use identifiable photographs and/or video of the participant on its website, social-media channels, showreels, promotional videos, online advertising and printed promotional material.NO: I do not give Studio Football permission to use identifiable photographs and/or video of the participant for its own marketing.

Marketing permission may be withdrawn by contacting play@studiofootball.co.uk. Withdrawal does not affect processing that was lawful before withdrawal.

8. FILE DELIVERY & 30-CALENDAR-DAY STORAGE

Raw and edited files are made available through Google Drive for 30 calendar days after delivery. The customer is responsible for downloading and safely storing their files during that period. After the 30-calendar-day delivery window, Studio Football will permanently delete the delivered raw and edited files from its Google Drive storage and cannot guarantee that they can be recovered or re-supplied.

9. SPORTS SCIENCE & SCOUTING INFORMATION

Where sports science testing, performance data or a scout-profile data sheet is provided, it is supplied for training, observational and informational use. Results can vary and do not guarantee a football trial, scouting interest, academy placement or professional recruitment.

10. LIABILITY & FINAL DECLARATION

Nothing in this waiver excludes or restricts Studio Football’s liability for death or personal injury resulting from its negligence, or any other liability which cannot lawfully be excluded or restricted.

2. MATCH PARTICIPATION, SAFETY & ACCEPTANCE OF RISK

Football match play involves physical exertion, changes of direction, tackling, contact with other players and other ordinary risks associated with the sport. Studio Football will take reasonable steps to provide a safe environment and manage the session appropriately, but not all risks of injury can be eliminated.

I understand that taking part in a football match involves inherent and ordinary risks associated with competitive football and physical sporting activity, including contact with other players, falls, collisions and injury. I voluntarily participate, or permit the participant named above to participate, on that basis.I agree that the participant will follow reasonable safety instructions and the rules of the game; wear suitable footwear and clothing; use the pitch and equipment appropriately; avoid reckless or deliberately dangerous play; report hazards or injuries; and stop playing if instructed. I understand that Studio Football may modify or stop play, or remove a participant from the match, where reasonably necessary for safety.I agree that the participant will play fairly and with reasonable consideration for the safety of other players, and will not engage in deliberately dangerous, aggressive or abusive behaviour. I understand that Studio Football may stop play or remove a participant from the match where reasonably necessary to protect the safety or welfare of others.I confirm, to the best of my knowledge, that the participant is fit to take part in football match play and that I have disclosed any relevant medical condition, injury, allergy or other information that Studio Football should reasonably know about for safe participation.

3. MEDICAL INFORMATION

5. FILMING

I understand and acknowledge that filming is an integral part of the Studio Football Match Day or Match Day Party experience and that the participant will be recorded during match play using video and/or photographic equipment for the purpose of producing the booked content and delivering it to the customer.

6. AI TRACKING AND FILMING LIMITATIONS

Studio Football uses automated AI tracking equipment as part of some filming setups. While this technology is designed to follow play and capture match action, Studio Football cannot guarantee 100% detection or capture of every ball touch, goal or individual moment.

7. OPTIONAL STUDIO FOOTBALL MARKETING PERMISSION

This choice is separate from core Match Day filming.

YES: I give Studio Football permission to use identifiable footage of the participant for its own website, social media, showreels, promotional videos, online advertising and printed promotional material.NO: I do not give Studio Football permission to use identifiable footage of the participant for its own marketing.

Selecting NO does not remove the participant from the match recording or customer-delivered match footage. Studio Football will take reasonable steps not to intentionally select or feature identifiable footage of that participant for its own promotional marketing.

8. FILE DELIVERY & 30-CALENDAR-DAY STORAGE

9. LIABILITY & FINAL DECLARATION
